Asbestos register and management plan. A printable asbestos register and management plan pro-forma — what is known to be present or presumed, its condition, who has been told and what happens before any work starts.
This records what a survey found and what has been decided about it. Surveying, sampling, assessing and removing asbestos is work for competent and, where required, licensed people — neither this sheet nor FacilityOptix does any of it. What the platform does is make sure the register is in front of a contractor before they start, rather than in a drawer.

What is on the sheet
- The survey or surveys relied on, their type, date, scope and anything they did not cover
- A row per material: location, type, extent, condition, and whether it is confirmed or presumed
- Risk assessment and the management decision for each — leave and monitor, encapsulate, or remove
- The monitoring record: who inspects, what they found and when they are next due
- Who has been given the register: contractors, engineers, cleaners and anybody who may disturb the fabric
- Arrangements before work starts, and what happens if something unexpected is found
Why this record matters
The duty to manage asbestos in non-domestic premises is a duty to find out, record, assess, plan and tell people — and the part most often missing is the telling. A register that lives in a drawer in the FM office is not a management plan, and the person who needs it is the engineer about to drill a wall at eight on a Saturday.
'No access' areas are the most important entries in any register and the ones people skim past. A survey that could not get into a riser has not found nothing there; it has found nothing out, and the correct entry is 'presumed' rather than a blank.
Condition and monitoring are what make this a plan rather than an inventory. Asbestos in good condition, undisturbed and managed, is a legitimate outcome; the thing that makes it legitimate is that somebody is looking at it on a known basis and recording what they see.
This is a soft download and emphatically so. Surveying, sampling, assessing and deciding what to do about asbestos is licensed and competent work, and this form is the structure for recording decisions somebody qualified has already made.
